Medicare is the US federal health insurance program for people 65+, certain disabled individuals, and those with end-stage renal disease.

### Payroll Tax
1.45% employee + 1.45% employer on all earnings (no cap). High earners pay an additional 0.9% above $200,000 (single) or $250,000 (married).
### IRMAA
Income-Related Monthly Adjustment Amount (IRMAA) increases Part B and D premiums for high-income retirees. Singles earning above $106,000 and couples above $212,000 pay surcharges up to 3.4× the standard premium.
